Practice Information

Locations

We currently have three locations in order to best serve the greater Toronto area. The central clinic is located in the heart of Forest Hill (439 Spadina Road, Suite 310). The office is accessible by public transit (walking distance from St. Clair West, or St. Clair subway station and St. Clair streetcar). Parking is available on the street and in a nearby parking lot. Our second clinic location is in downtown Toronto at Queen and University (250 University Avenue, Suites 213 and 214), accessible by public transit (Osgoode subway station or the Queen streetcar) with parking available in a nearby paid public parking lot on Simcoe St. Our northern GTA clinic location is on Keele St. in the Vaughan area (9131 Keele St., Suite 149). There is free parking available and the clinic is within walking distance from the Rutherford GO station.

Booking an Appointment

Professional referrals (e.g, family physician, psychiatrists) are accepted but not required – a person can self-refer to our assessment and treatment centre. An initial consultation can be booked by telephone (416-432-4587) or e-mail info@foresthillcbt.com.

OHIP and Insurance Coverage

Psychological services are not covered by OHIP, but are partially or fully covered by most extended health care plans if services are provided by registered psychologists. The psychologists at the Forest Hill Centre for CBT are registered with the College of Psychologists of Ontario.

Fees

Fees are charged on an hourly basis. Payment is accepted by cheque, cash, or e-transfer at the time of each session. We provide a detailed invoice following each session which can be submitted to insurance companies for reimbursement (if applicable).

Cancelled or Missed Appointments

To get the most benefit from CBT, it is very important to attend regularly scheduled appointments. If you must change or cancel an appointment time that your psychologist has set aside for you, it is important that you notify your psychologist at least 48 hours in advance. Except under certain extenuating circumstances, you will be charged the full hourly rate for an appointment that is not cancelled 48 hours in advance.

Emergencies Experienced During Treatment

In case of emergency you should go to the emergency room of the closest hospital.
